When flying a direct flight from Frankfort, Germany to Cincinnati,OH. Which airport do you go through customs?

If your going direct you'll land at Cincinnati (or Northern Kentucky) international airport
Both, being an international flight, but the destination generally has forms that are required to be completed.
If you're not going direct, it really depends on the airline you're using. Different airlines have different hubs and you will go through customs in the first American airport you land in.
Customs is most often cleared at the Departure airport these days. But Immigration is usually cleared at the Destination airport.
You should be going through customs at CVG. Just so you know, CVG is located in Northern Kentucky so don't worry if the flight attendant says that the airplane is landing at Northern Kentucky International Airport. Apparently some passengers have thought they boarded the wrong plane when coming to our city.
You'll go through both when entering the respective countries.
